  1. Accessing or using this website and the information therein constitutes acceptance of the Terms and Conditions of Facility Use below. Oral representations or agreements not set forth in this agreement are not binding. This agreement supersedes any preceding agreement between the parties relating to the services of this website.
  2. The accuracy, authenticity, security, and confidentiality of the information submitted on this website is the sole responsibility of the facility submitting the information. The use of this Terms and Conditions of Facility Use does not, in any manner whatsoever, add, diminish or otherwise change the obligations between the facility and the service provider, or supersede, replace, or supplement, any Terms or Conditions contained in any written contract or documentation disseminated by the service provider in any other manner, other than through the website. In the event of a conflict between these Terms and Conditions of Facility use, unless indicated otherwise, the aforementioned shall control, including documents published and mailed to your facility. In the event of conflict between these Terms and Condition of Facility use and a specific end user agreement, then the end user agreement shall control.
  3. By use of this site you
    1. represent that you are a duly authorized representative of the Facility
    2. agree that you abide, and will continue to abide to all Terms and Conditions regarding Privacy and security as defined by the most recent HIPAA/HITECH government regulations and
    3. agree to the following:
      • that you shall provide access to this website only to those individuals, including employees, agents and contractors (collectively referred to as the "workforce") who need it to perform their jobs.
      • that you and the workforce understand that the information, viewable and printable online within the application, shall be handled in the same confidential manner as other medical and personal information is handled in the facility.
      • that this information may be shared only with the patient or a duly authorized representative of the patient.
      • that you and the workforce accessing patient information exercise due care to log off or lock workstations if they will be left unattended for long periods of time.
      • that you and the workforce accessing patient information may not share logon IDs or passwords and shall exercise due care to safeguard logon ID and password to ensure no unauthorized persons may log on to the system.
      • that you (for "Super users" only) will terminate access capabilities of your workforce no longer working for the facility or whose job functions do not require access, or of those individuals no longer associated with the facility.
